Pano po ba ang dapat kong gawin sa birth certificate ko, mula pagkabata ko hanggang magkatrabaho ako gamit ko ang apelyido ng nanay ko ngayon lumabas sa nso ko na nasa apelyido ako ng tatay ko at nilagay ng aking ina na kasal sila ngunit hindi talaga. Gusto ko sanang maayos ang birth certificate ko sa pangalan ng nanay ko, paano po ba ang dapat gawin? Pati mga anak ko gamit din ang ang apelyido ko sa nanay ko.

Hindi mo sinabi if ang birth certificate sa local Civil Registry ay apelyido ba ng nanay mo ang nakarehistro at conflict ito sa PSA?
Maaring mong sundin ang surname ng tatay mo based in PSA at maiprocess ito through administrative (Civil Registry) if you are born before August 3, 1988 kailangan mo ng Authentic writing and Other supporting documents that would prove the paternity of the father over the child. Ibig sabihin kailangan mo ng mga ebidensya na kinikilala kayo ng iyong ama kahit na hindi sila kasal.
If born after August 3, 1988, The affidavit of acknowledgment executed by the father should be registered with the civil registry office where the birth of the child was registered. Since the surname being used is the surname of the mother, an Affidavit to Use the Surname of the Father (AUSF) should also be executed with the civil registry office where the birth is registered.
However, if gusto mong gamitin ang apelyido ng ina habang buhay considering na ang issue ay relating to the change of surname or apelyido at hindi na ito covered as clerical/typographical error. Kailangan mo kasing ipacancel or ipaannul yong family name ng tatay mo dahil hindi naman sila kasal. Kailangan mong magpetitisyon sa korte nito at you need the services of a lawyer para sa pagfile ng court.
