How can my baby have his father's last name or surname? Apelyido ng tatay

I would like to ask a question: My baby will be born soon..and I'm planning on giving the child his father's last name.. Pero hindi pa po kami kasal.. and as of this time yung father ng baby ko is outside the country ngayon.. 2 years pa bago sya makauwi.. Paano po ang magiging proseso sa pag fill out ng birth certificate form na ibibigay sakin ng ospital? Pwede ko na po ba ilagay ang name ng baby ko na naka-apelyido na sa father nya?..Anu anu po ba yung kailangan document para mailagay ko sa magiging birth certificate ng baby ko ang father nya..??

Translation: My baby will be born soon and I am planning to let my child use his father's last name. But we are not married and the father is currently abroad. He will be back in the Philippines after 2 years. How can I process the birth certificate form etc., that would be given to me by the hospital? Can I put the father's surname on the birth certificate form? What documents are needed so that I can name the father of my child on the birth certificate?

Kung hindi kayo kasal, puede pa rin naman na ibigay sa baby ang last name ng father basta’t sa likod ng birth certificate, iyong affidavit of recognition, pipirma ang father. Ang advice ko, don’t have the child registered until the father has signed the birth certificate. Kasi, kapag nagfile ka ng birth certificate tapos walang pirma ang father sa Affidavit of Recognition, hindi sila papayag sa NSO o sa Civil Registrar na ilagay ang bata sa pangalan ng ama. The NSO will ask you to have the father sign the Affidavit in the back before they allow you to register the child in the name of the father.
