I'm not sure I understand what you mean. Your mother -- how old is she? I think that she may have been born in a remote barrio. Her birth was registered by the kumadrona or the midwife, but the spelling of her name and her date of birth is different?

Or do you mean that your mother's birth was registered late but the registration of her birth did not match the name she uses or her date of birth?

Does your mother have a baptismal certificate from the Catholic church? That is a good document to show that your mother has been using the name she has been using because she did not know that her name in her birth certificate is different.

Her school and employment records? Her SSS? or GSIS?

Since hindi naman kayo naglagay ng mga payak na mga facts, paano ko naman ito masasagot? Mag-post ka ulit -- this time, add facts that are relevant. Ilagay mo ang kuwento ng buhay ng nanay mo at nang masabi ko kung ano ang dapat mong gawin.

Essentially, there are two actions that you can do: you can go to the NSO and say that the mistake was a clerical mistake only so it should be corrected by the NSO. Or, you might need to go to the RTC and file a case as a person's name might affect her other personal and civil rights. So, unless you put in all the relevant facts, I cannot answer you definitely.

Hi po Atty. 56 y/o na po ang mother ko and her name is NENITA and DEC.11,1957 her birthdate. opo Atty. kumadrona po ang nag-register ng birth certificate ng mother ko sa probinsya po iyon. Sa Birth Certificate po NATY po ang naka-pangalan at DEC.23,1957 naman po iyong birthdate at sa mother's name po instead of LIBRADA MONSES ay LIBRADA MONCEC po ang nakalagay. Sa marriage contract naman po sa upper part ay LITA at DEC.11,1959 naman po ang nakalagay and her father's name was entered as LUCIANO instead of GRACIANO and sa mother's name din po ay LIBRA MOISES instead of LIBRADA MONSES. sa baptismal certificate po tama naman po iyong pangalan NENITA iyong birth date po ang nakalagay po ay DEC.16,1957. Hindi po member ang mother ko sa SSS at GSIS pero may ID naman po ang mother ko POSTAL po nakapangalan naman po iyon sa NENITA. Gusto ko po sana Atty. ayusin ko na po iyong mga error sa mga documents po na iyon..., iyon nga lang po hindi ko po kasi alam kung paano po iyong first step. Naku, marami palang mga entries na mali. Pero, ang kabutihan nito, puro mga typographical errors lang. Ito ay puedeng mai-correct administratively sa NSO. Kailangan kang pumunta sa NSO. Kailangan mong kausapin iyong information officer doon sa pagkat sila ang makapag-bibigay sa inyo noong checklist nang mga dapat ninyong gawin.

Marahil ay kakailanganin ng nanay ninyo ng affidavit noong kumadrona (I hope that the midwife who birthed her is still alive). If the kumadrona is no longer alive, you must find two relatives of your mother who are older than her and who have known her since bata pa siya, preferably, since she was born. They will attest to who her parents are, when she was born and what name her parents gave her. This will document the mistake that the midwife made.

Tapos, kung may diploma o report card ang iyong ina, na nakalagay doon ang ginagamit niyang pangalan, ang kanyang marriage certificate, ang mga certificate of live birth ng kanyang mga anak, iyon yung mga supporting documents na magpapakita ng continuity ng gamit niya nooong NENITA.

I think that you will have to file a Petition sa NSO to correct a typographical error. May bayad din iyon. Nasa checklist nila iyon. Good luck sa iyo.
