Kailan po ba kayo ikinasal? You did not mention if yong birth certificate na sinasabi mo ay local or PSA na. So, if in your birth certificate issued by PSA , dala mo ang surname ng tatay mo, ay correct yan dapat ang sundin mo. Maari po kayong pumunta sa Local Civil Registrar if pwede pa po itong macorrect by filing petition in their office lang po dahil kung hindi sa korte ka na magpetitisyon tungkol dito and you need the assistance of the lawyer para sa pagpafile ng petitisyon. Maintime gather all supporting documents to established sa kung ano ang tama na apelyido mo.

I was married July 5, 2015 , sa NSO ko na nakuha last year po ay apelido ng mother ko sa pagkadalaga ang nakalagay pero since birth dala ko po apelido ng father ko not knowing na mothers maiden pala ang nakalagay sa NSO.
Pero i was married using my fathers surname

Kailangan patunayan at may evidence ka about this.
