Normal amylase but 2x upper limit value of lipase.He have high triglycerides with 300 mg/dl and sgpt of 80U/L,but took medication of lipway 250 mg and silymarin 140 mg,but still have triglycerides of 282 mg/dl for almost 3 months.What best explain this condition?

Good morning!

High lipase values indicate problem with the pancreas or the ducts that drain it. It might also explain why you have problem with triglycerides and sgpt as the pancreas is connected to the liver which produces triglycerides and sgpt.

Several reasons might explain this. One example is if you have a blockage in the the tubes which may be caused by gallstones. Another scenario is for you to have a mass inside or outside the tube that impinges the flow of the product of the liver and pancreas.

the fatty liver explains the high cholesterol. The large amount of residual urine possibly explains an enlargement of the prostate (if the patient is male)

Anyway, going back, since there's a normal abdominal ultrasound, then perhaps the main problem is the pancreas itself. Acute pancreatitis may be caused by alcohol intake and biliary microlothiases (small gallstones which were not visualized by the ultrasound)

is it treatable?and if pancreatitis (if its underlying really a cause), cured and became normal lipase pancreas function, is anti hypertriglyceridemia will by now cure by medication that i have took for several months (lipway 250 mg to be specific)?

Acute pancreatitis is something transient.

Fenofibrates are drugs specific for lowering triglycerides. However, if there seems to be no change in your trigly level after 3 months then it would be wise for you to report that to your attending physician so he can adjust your meds.
